Covid-19 Screening Questions

• Do you have a fever, cough or any symptoms of COVID-19?

• Have you traveled outside of Canada in the last 14 days? 
  
  *If you answer yes to any of these questions, please call to reschedule your appointment.

1. Please arrive alone.  If you are under 18 years old, need a translator, or have mobility issues, you can bring one other person to accompany you.

2. Masks are required to enter the clinic.

3. Please arrive no more than 5 minutes before the scheduled appointment.

4. Social distancing will be practiced at all times.

5. Payment with card is preferred over cash.
